A referral can come from:
– Your GP,
– A medical specialist (for example, neurologist, geriatrician, psychiatrist, paediatrician), or
– Another relevant professional involved in your care (for example, a rehabilitation provider or case manager), if agreed in advance.
The referral should explain:
– The reason for the assessment (for example, memory concerns, diagnostic clarification, capacity, return-to-work questions).
– Relevant medical history and test results, including imaging where available.
– Any specific questions that need to be answered in the report.
Having a clear referral helps us design the right assessment and produce a report that is genuinely useful for you and your treating team.
