How do I request a letter, report or other documents?

Your clinician can provide a range of written documents, such as:
– Brief letters to your GP, specialist, school, or employer
– Treatment summaries for referrers or other health professionals
– More detailed clinical or neuropsychological reports, where appropriate

To request a document:
– Discuss the purpose with your therapist in session so they understand what is needed and whether it is clinically appropriate.
– Current clients can then either discuss the details in session or complete our online Document Request Form between appointments. The form is for existing clients only and is reviewed during business hours.
– Allow adequate notice. Simple letters may be completed within 1–2 weeks; detailed reports usually require longer lead times.
– Be aware of fees. Most written work attracts a fee because it involves additional time outside your appointment. The fee depends on the type and length of the document.

If you have a deadline (for example, a meeting, legal matter, or funding review), please let us know as early as possible so we can advise whether the timeframe is realistic.
